Job Description
JOB FUNCTIONS:
- Ensure that all casework services are consistent with the operating policies of the Office of Community Corrections and other regulatory agencies.
- Promotes and leads agency culture of learning and respect to others as well as agency mission and strategic plan while empowering staff in meeting the agency mission.
- Ensure that participant documents, such as a copy of all intake materials, are placed in participant files, including those provided by supervising agencies (e.g. Probation, Parole and Sheriffs Department).
- Complete client intake assessments utilizing assessment tools (e.g LS/RNR, TCUDS, etc); assess risks/ responsivity needs; and develop treatment plans for each assigned participant.
- Ensure that treatment plans and goals for transition are completed and reviewed on a monthly basis. Provide feedback to participants.
- Review and finalize weekly Utilization Report.
- Make necessary referrals to outside agencies or departments as needed. Case manage community based services.
- Complete regular check-ins with participants.
- Document participant interaction (i.e., client notes) as needed.
- Assign and track completion of CBIs (Client Behavioral Interventions) as needed.
- Create an after care / discharge plan for each assigned participant.
- Facilitate CBT manualzed groups with fidelity. Submit to / conduct group audits.
- Utilize technology for programming needs (i.e., virtual meetings/ groups or hybrid models).
- Keep participant data and information updated in the system (Client Information Management System or CIMS).
- Audit clinical files and oversee process for making corrections when necessary.
- Transport or conduct drug testing on participants upon staffing deficiencies.
- Participate in all meetings and trainings as requested by the supervisor(s).
- Facilitate clinical meetings and provide supervision to staff during the absence of supervisory staff.
- Assist Director in interviewing and training new staff as needed.
- Participate in program development or other administrative activities as needed.
- Learn and demonstrate respect for the roles of other agencies (OCC, Sheriff's Department, Parole, Probation, etc.).
- Actively participate in annual performance evaluation process at the Center.
- Perform other job responsibililties as directed by the supervisor(s).
Education/Experience/Physical Requirements:
- Bachelors degree required. Master's degree in Clinical Psychology, Education, Counseling, Psychology, Rehabilitative Counseling or Social Work preferred.
- Minimum of two (2) years of supervised counseling experience.
- Licensed by professional board of practice in counseling, mental health, social work, or alcohol and drug abuse preferred.
- Previous management experience strongly preferred.
- Experience delivering manualized cognitive behavioral treatment, in a group setting, utilizing evidence based CBT curriculum preferred.
- Must be willing to work with high risk populations including substance users, justice involved individuals and trauma survivors.
- Strong oral, written, and presentation skills.
- Computer proficiency a must that includes Microsoft Word, Excel and Power Point.
- Some evening availability required.
- Some traveling required for meetings and trainings.
- Must have a valid driver's license in good standing.
- Must submit and pass drug testing upon hire.
- Must be able to sit, stand, bend and walk during the work day.
Closing Statement: To enhance the overall operation of the Old Colony Y and support team work, the Case Manager will accept additional responsibilities as assigned.
