Job Description: Help Build Something Meaningful With Us As a Behavioral Health RN Case Manager in the Palmer Territory with Elara Caring, you will provide compassionate, individualized care to behavioral health patients in their homes. You will manage a dedicated caseload, conduct assessments, develop care plans, and coordinate interdisciplinary care, helping patients achieve better health outcomes in the comfort of their own environment.
