Summary

INTRODUCTION Under the supervision of the VP of Grants, the Program Manager (Chronic Care) directly assists with developing programmatic infrastructure related to chronic care health services such as the Diabetes Prevention Program (DPP), Diabetes Self-Management Education and Support Program (DSMES) and the Hypertension Initiatives; as well as supporting the development and implementation of policies and procedures relevant to managing these populations. This position is instrumental to the administrative management and coordination of Unity’s Chronic Care programs including but not limited to: preparation and dissemination of regular program updates, writing and preparing proposals and reports, budget management, contracts management, and data monitoring. The Program Manager interacts both with internal team members and external partners and stakeholders, including sub-recipients, consultants, community-based partners, government representatives, and the general community. This position is an integrated member of Unity’s Chronic Care team. MAJOR DUTIES/ESSENTIAL FUNCTIONS Essential and other important responsibilities and duties may include, but are not limited to the following: Responsible for oversight and management of non-clinical aspects of Chronic Care Programs. Collaborates with appropriate Discipline Directors and Chronic Care leadership to assess, formulate, implement, manage and monitor Unity’s patient centered chronic care programs and services. Responsible for compliance monitoring for all federal, state, local requirements related to Unity’s chronic care programs. Leads team(s) in annual goals setting that align with Unity’s organizational goals and national recommendations. Leads team(s) in determining effective strategies to achieve annual goals and monitor department’s work plan. Seeks, applies for, and manages funding sources. Prepares proposals, progress and annual reports, and facilitates program evaluation activities. Develops and monitors budgets; ensures appropriate expenditure allocation. Analyzes data trends; performance evaluations, and prompts team discussion for quality improvement/training strategies. Prepares Unity for federal and local program related audits and visits related to the chronic care program. Serves as primary contact for HHS/HRSA, DOH, and/or other local and or national partners/stakeholders. Represents Unity’s Chronic Care programs at local, regional, and national events. Builds and maintains relationships with local and national partners/stakeholders. Ensures ongoing communication inter-departmental stakeholders such as discipline directors and clinical coordinators to support coordination across the chronic care team. Participates in the interviewing and/or training of non-clinical staff and interns/volunteers, when appropriate. Strives to maintain updated knowledge on all existing and emerging topics and the relevant political landscape. Attends operational and quality improvement meetings and/or committees to ensure program integration, and engages in creating and maintaining a culture of excellence. Engages in ongoing communication about program progress, including successes and challenges with supervisor. Participates in strategic and operational planning by recommending programmatic and service priorities and opportunities. Performs other duties as assigned. MINIMUM QUALIFICATIONS Master’s Degree (Public Health or Health Care Administration, preferred) from an accredited college or university in a related field of study OR an equivalent combination of education and experience which provides the required knowledge, skills and abilities deemed sufficient to prepare the applicant to successfully perform the duties of the position. Two (2) years of professional program management experience in a community-based health program. Experience with grant writing and program evaluation a plus. REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES Strong understanding of the concepts and practices associated with effective program management. Must have a high degree of customer service skills and philosophy to include professionalism, responsiveness, and follow-through. Demonstrated ability to maintains a high level of ethical conduct regarding confidentiality, relationships, and professional stature. Demonstrated ability to handle multiple tasks effectively to meet deadlines and strong organizational skills and attention to detail. Demonstrated ability to show a high degree of initiative and proactive planning and learning Demonstrated ability to work with a multi-cultural and multidisciplinary team. Ability to analyze and systematically compile technical and statistical information. Must be able to prepare reports and correspondence. Ability to comprehend and make inferences from written material. Ability to interpret federal, state, and local government laws and regulations. Strong organizational skills and attention to detail and accuracy. Demonstrated ability to effectively work and communicate with diverse populations. Demonstrated proficiency with technology and business software (e.g. Microsoft Office Suite). SUPERVISORY CONTROLS The Program Manager (Chronic Care) reports to the VP of Grants. The Program Manager has delegated authority to implement approved plans and make implementation decisions in coordination with the chronic care team. The position will consult with the VP of Grants before deviating from the approved plans or making decisions that will significantly affect program goals, resource allocations, or other health center operations. The Program Manager will analyze options and make recommendations for decisions by the VP on proposed goals, plans and budgets. GUIDELINES This position abides by all rules and regulations set forth by applicable licensing and regulatory bodies, as well as UHC policies and Procedures. PERSONAL CONTACTS The position requires contact with staff at all levels throughout the organization. There are also external organizational relationships that may be a part of the work of this individual. PHYSICAL EFFORT AND WORK ENVIRONMENT The work is basically sedentary in nature. However, some walking, bending, and the carrying of light items is required. Refer to the attached ADA checklist. OTHER SIGNIFICANT FACTS The incumbent must be able to balance the needs of diverse constituencies on a daily basis. RISKS The position involves everyday risk and discomforts, which require normal safety precautions typical of such places as offices, meetings, training rooms, and other UHC health Care Sites. The work area is adequately lit, heated, and ventilated. All medical services shall be provided according to medically accepted community standards of care. The employee shall provide evidence of a recent (within the past twelve (12) months) health assessment that includes a PPD and/or chest x-ray results.
