About The Role
As a case manager on the High-Risk Youth Intervention Services (HRYIS) Team, the case manager will serve high-risk youth in identified Chicago communities with highly concentrated firearm violence. This role provides trauma-informed assessments and case management services to youth in the office and community settings. Interventions include a range of behavioral services for youth and their families with histories of trauma and mental health needs. Services are flexible to the client's and family's needs. The case manager will provide services in a culturally sensitive and appropriate manner.
What You Will Do
Deliver individualized assessment-based case management services to at-risk or high-risk youth to school-age youth 6 to 17 years old and their families
Provide high-risk youth with evidence-based intervention services that reduce involvement in the criminal and juvenile system, increase school attendance, and when necessary, refer high-risk youth into therapeutic programs that address trauma recovery and other mental
health improvements
Administer the Youth Assessment and Screening Instrument (YASI) to each program participant to assess participant's risk and protective factors
Complete intakes, assessments, develop case plans, and provide or connect the individuals to identified resources and services
Conduct mental health assessments for youth assessed to need a mental health assessment
Continually assess for any safety concerns and facilitate safety planning when indicated
Actively outreach and collaborate with other providers involved in the identified client's life such as teachers, case managers, parole officers, etc.
Provide culturally appropriate, competent, and individualized supervision in accordance with each supervisee's age, gender and gender identity, sexual orientation, race, ethnicity, religious beliefs, and culture
Work cooperatively with other members of the treatment team and participate in weekly team meetings
Attend weekly supervision
Provide weekly, individual reflective supervision to each supervisee
Maintain positive and mutually respectful relationships with other human service providers, entitlement agencies, health care vendors, and the community
Assist with outcome measures
Maintain compliance with clinical documentation, timelines and agency policies and procedures including meeting required timeframes for documentation and service notes
Actively seek consultation whenever necessary
Report to assigned supervisor and actively seek consultation whenever necessary or requested by supervisor
Develop close working relationships with DCFS and DMH-funded agencies, and other mental health service providers
Maintain compliance with confidentiality policies and procedures
Participate in training opportunities
Other duties as assigned by supervisor
Our Ideal Candidate
Education and Experience:
BA/BS in psychology or a related field is required
Excellent interpersonal, verbal, and written communication skills
Personal vehicle, valid Illinois driver's license, and insurable driving record is required
