As a Home Health Nurse or RN, you will be responsible for traveling to several homes a day to administer healthcare services and help patients maintain their independence. Duties may include administering at home IVs, changing dressings, cleaning wounds, and foley/catheter insertions.
Responsibilities
Case management and supervised visits.
Perform skilled and comprehensive assessment of the patient during the initial admission.
Assess and chart observations of the patient’s condition at each visit (vital signs.
Provide skilled nursing care as needed in accordance with the plan of care (administering medication, cleaning/dressing wounds, assessing the healing progress).
Provide proper patient education for home health care procedures and strategies.
Responsible for the overall clinical record maintenance to include visit note (must be completed within 24 hours), comprehensive assessments, including OASIS (must be completed within 48 hours), medication profile, orders/plan submitted in a timely manner, etc.
Maintain confidentiality of patient PHI through HIPPA law.
Keep the patient comfortable through palliative care.
Qualifications
Current Texas State license as a Registered Nurse.
2+ years' experience as a RN in Home Health.
Current CPR certification.
Reliable transportation.
Texas Driver's License and Auto Liability Insurance.
Current TB Skin Test
EMR experience (Kinnser preferred).
