Summary

Job Title: Targeted Case Manager – Behavioral Health Location: 1400 NE 125 Street, North Miami, Fl. 33161 Position Type: Full-time / Part-time Job Summary: We are seeking a highly motivated and compassionate Targeted Case Manager to join our behavioral health team. The Targeted Case Manager will be responsible for providing comprehensive case management services to clients with mental health and/or substance use disorders. The primary goal is to assist clients in gaining access to necessary medical, social, educational, and other services, helping them achieve recovery and improve their quality of life. This position involves working closely with a multidisciplinary team to ensure continuity of care and support for our clients. Key Responsibilities: Conduct initial assessments to determine clients’ needs, strengths, and barriers to accessing services. Develop, implement, and monitor individualized service plans (ISPs) in collaboration with clients and their families. Assist clients in accessing behavioral health services, medical care, housing, employment, education, and other community resources. Coordinate services with healthcare providers, social workers, and other professionals involved in the client’s care. Advocate for clients’ needs within various systems, including healthcare, education, housing, and legal services. Provide ongoing support, monitoring progress, and making adjustments to service plans as needed. Conduct regular home, community, or office visits to assess progress and address emerging needs. Facilitate communication and collaboration between clients, family members, service providers, and community organizations. Maintain accurate, up-to-date documentation in accordance with agency standards and Florida Medicaid guidelines. Participate in interdisciplinary team meetings, case reviews, and supervision to ensure quality and continuity of care. Assist clients in understanding their rights, navigating the healthcare system, and overcoming barriers to accessing services. Qualifications: Bachelor’s degree in Social Work, Psychology, Counseling, or a related field required (Master’s degree preferred). Minimum of one year of experience working in case management, mental health, or substance use treatment. Certification as a Targeted Case Manager (TCM) in Florida or the ability to obtain certification. Knowledge of Florida Medicaid guidelines and state regulations regarding case management services. Strong understanding of community resources, service coordination, and advocacy. Ability to work independently while maintaining a collaborative approach with the care team. Excellent organizational, communication, and interpersonal skills. Proficiency in using electronic health records (EHR) and documenting case management services. Preferred Qualifications: Experience working with individuals with co-occurring mental health and substance use disorders. Knowledge of trauma-informed care and recovery-oriented approaches.
