Targeted Case Manager: Required: Bachelor’s Degree in related field: Social Services, Human Services, Health or Education. 1. To provide information, support and/or intervention, whenever possible and clinically appropriate to the members and families to ensure continuity and consistency of treatment in a timely manner. 2. Remain aware of resources available to meet the clients’ needs and link client to services as needed. Assist client with community integration by participating in team meetings. 3. Assess client needs and create treatment plan and update as needed. Maintain appropriate documentation of all services provided. 4. Act as an ambassador for CACC with professional conduct in a positive manner within the community. Establish and maintain interpersonal relationships; develop constructive and cooperative working relationships with others and maintain them.
