Substance Use Disorder Treatment Coordinator
Summary of Position:
Works closely with outpatient providers in providing coordination of substance use treatment and mental health services to persons with co-occurring disorders in order to obtain best outcomes.
Conducts comprehensive drug and alcohol assessments using evidence-based tools (ASAM, SBIRT, etc.).
Engages client in comprehensive bio-psychosocial assessment of their strengths, needs, and circumstances for the purpose of identifying gaps in care for clients with co-occurring disorders, and provides linkage to appropriate services to close these gaps.
Assists client in identifying and assessing informal and formal supports and resource systems, including but not limited to, social, recreational, family, spiritual, and neighborhood resources.
Assists client in identifying, accessing, and negotiating formal service systems in the community, including substance use and mental health treatment services, medical healthcare, housing, income/ employment, educational, social service, and other related services.
Qualifications/Requirements:
Good working knowledge of substance use disorder services, community resources and how to access these resources.
Ability to work with a diverse population.
Ability to work effectively with an interdisciplinary team and persons from different agencies and providers.
Ability to adhere to agency policies of confidentiality, ethics and mandatory abuse reporting.
Bachelor's degree in social work and/or criminal justice or in related area.
CADC or ACADC required.
Previous experience in working with persons with co-occurring substance use and mental illness
Previous experience in utilizing the ASAM preferred.
