The Job: Provide case management services under a VA-funded Housing First grant, aimed at achieving housing stability for Veteran households. This role is responsible for establishing and maintaining linkages with VA and community-based resources, coordinating collaborative community efforts, and advocating on behalf of participants in Harbor Care's Supportive Services for Veteran Families (SSVF) program.

Duties & Responsibilities
*
Provide strengths-based case management to assigned SSVF households. Services include, but are not limited to: screening, intake, assessment, development of Housing Stability Plans, budgeting, ongoing monitoring, connection to appropriate community resources, follow-up, advocacy, appropriate discharge planning, and outcome tracking.
* Monitor participant progress on Housing Stability Plans and update or extend plans as needed to support housing stabilization goals.
* Maintain accurate and timely documentation of all case management activities, including case notes, client files, and entries into the Homeless Management Information System (HMIS).
* Adhere to the policies and procedures outlined in the SSVF Program Manual.
* Uphold strict client confidentiality standards at all times.
* Represent Harbor Care and coordinate with internal and external service providers to support participant needs.
* Engage in ongoing professional development and stay current on best practices related to veterans and homelessness.
* Attend all SSVF team meetings, relevant regional coordination meetings, webinars, and trainings.
* Conduct outreach to identify veterans experiencing homelessness and connect them with housing resources.
* Maintain an up-to-date calendar of professional activities in Outlook.
* Coordinate with the Outreach Lead, Community Data Specialist, and Housing Navigator to effectively address veteran needs.

* Ability to meet with households with diverse needs and challenges
* Display a high level of initiative, effort and commitment towards completing assignments efficiently.
* Able to meet a flexible work schedule
* Be aware of and adhere to all HIPAA rules and regulations and must be an active participant in safety measures for ensuring confidentiality of information as it relates to clients and this agency
* Ability to work independently
* Clean Driving Record

Educational and Professional Requirements:

Associate's degree or Bachelor's degree in social work, Behavioral Science, or a related field preferred.
* One to two years of prior experience in adult case management.
* Familiarity with community resources related to homelessness and veteran services.
* Experience working with vulnerable and/or homeless populations.
* Proficient in Microsoft Office applications (Word, Excel, PowerPoint), Outlook, and Internet-based tools.
