Responsibilities and Duties:
Assess patient needs and develop individualized treatment plans based on physician orders and interdisciplinary team input.
Provide direct speech therapy services to patients in their homes or assisted living facilities within a defined geographic area.
Ensure timely documentation and compliance with all Paradigm HomeCare policies and procedures.
Collaborate with physicians, nurses, and the care team to support optimal patient outcomes.
Qualifications & Skills:
Master's Degree in SpeechLanguage Pathology from an accredited university
Active SpeechLanguage Pathologist license in the State of Texas
Current CPR certification
Valid Texas Driver's License and automobile insurance
Minimum 3 years of experience as a Speech Language Pathologist, with at least 1 year of recent home health experience (within the last three years)
Knowledge of WellSky EMR software desired
