Job Description
This position is responsible for conducting psychosocial evaluations and management for both the adult and geriatric patients, participating in team review of patient progress recommending changes in the treatment based on the patient’s current, physical and psychosocial needs, and providing casework services to patients and their caregivers in dealing with end stage renal disease.
Education And Credentials
Masters degree in Social Work from an accredited program. New York State licensure required. BLS required within 90-days of start date. Crisis Prevention Intervention training required within 12-months of start date.
Experience
Two (2) years experience in a hospital setting or related social agency preferred. Assignments require some investigation and some independent judgment.
