Position Description
The Social Worker is responsible for providing counseling and case management services to Chinese, Spanish
and Arabic-speaking immigrant populations in Sunset Park, Brooklyn. In addition to offering regular social work
counseling, AMPHS’ unique, integrated programming model also enables community members to receive direct
social service counseling during seasonal community screening events and in coordination with our literacy
programs. Social work services are currently being offered in-person and remotely.
Primary Responsibilities:
Provide support in healthcare navigation and care coordination.
Provide culturally-appropriate direct services, such as social work counseling and case management,
working closely with AMPHS health services team to address social determinants of health.
Provide ongoing follow-up, motivational interviewing, and goal-setting with community members, while
monitoring and evaluating progress and modifying treatment plans accordingly.
Coach community members in effective management of their chronic health conditions and self-care.
Coordinate with community partners to advocate for families and refer to specialized services, such as
medical and mental health care, legal services, educational programs, and nutritional assistance.
Develop literature for social service outreach and education.
Facilitate workshops and support groups (e.g. self-care support group, health access workshops, etc.)
Assess and intervene in domestic violence and child abuse situations.
Manage a regularly assigned caseload of community members at AMPHS open hours and record care
management information in EMR system no later than 24 hours after contact.
May be required to perform other duties as assigned.
Minimum Qualifications:
LMSW or LCSW required.
Oral and written fluency in Chinese or Spanish and English required.
Prior social work experience in a multicultural, community-based setting for at least 1-2 years preferred.
Basic computer skills and knowledge in Microsoft software.
Good communication skills, such as listening well, and using language appropriately.
Ability and willingness to provide emotional support and motivation to patients.
Compensation:
Annual Salary Range: $60,000 - 65,000, commensurate with experience
