Job Summary: Provides psychosocial services and community resource referrals to patients and families and meets for consultations as needed. Acts as a competent clinician who provides clinical care in a skilled and effective manner, utilizing appropriate technical and decision-making skills. Job Requirements: Education and Work Experience: * Bachelor's Degree or equivalent (some certifications will require Bachelor's): Required * Master's Degree in social work or equivalent combination of education/experience: Preferred * Experience in a hospital or healthcare environment: Preferred Licenses/Certifications: * Licensed Clinical Social Worker (LCSW): Preferred Essential Functions: * Demonstrates knowledge of and ability to care for high-acuity patients. Initiates a plan of care and appropriate patient teaching. Reassesses plan of care according to the physical and emotional needs of patient. * Develops social work Plan of Care (POC) based on patient assessment including, psychosocial factors that would impede the POC, clinical knowledge of appropriate theories of stages of human development, the impact of terminal illness, mental status and family dynamics. * Identifies psychosocial strengths and weaknesses, including coping skills, grief reactions, support systems, high risk factors for complicated bereavement and the need for community resources * Provides direct social work services to patients who have problems with drug and alcohol abuse and/or mental problems. * Provides therapeutic counseling and intervention to mobilize patient and/or patient caregiver toward achieving positive outcomes. Also provides crisis intervention to patients/families in emergent need. * Performs other job-related duties as assigned.
