The Organization

The Population Health Learning Center (PHLC) was established in 2023 to address California’s critical need for a more equitable and efficient healthcare system. California's diverse population and fragmented healthcare infrastructure contribute to persistent inequities, particularly among low-income communities. PHLC was created to bridge these gaps through an innovative, scalable approach that aligns policy, technical assistance, and practice transformation to drive improvements in population health and health equity across the state.

PHLC strives to become a central hub for Population Health Management (PHM) and Value Based Care (VBC) innovation. To achieve this, the Learning Center (LC) oversees a variety of programs and initiatives designed to strengthen PHM and VBC capabilities. Our flagship initiative is the statewide Medi-Cal-focused Equity and Practice Transformation Initiative (EPT), a $140 million investment through 2026. In addition, the Learning Center is collaborating with multiple funders on related population health transformation projects, leveraging our existing EPT infrastructure to enhance PHM capabilities. This includes utilizing data and technology-driven workflows to improve care, addressing behavioral health and social needs through effective screening and response, and supporting the adoption of efficient team-based care models. The Learning Center is a fiscally sponsored project of the Tides Center, a 501(c)(3) public charity organization that fiscally sponsors over 160 projects. This work aligns with Tides’ vision, in that health equity is central to enabling all populations to thrive and to share prosperity.

The Position

The Senior Program Manager at the Learning Center will take the lead in developing and implementing PHLC's new programs, focusing on key areas of Population Health Management (PHM). This includes utilizing data and technology to improve patient outcomes and reduce disparities, collaborating with managed care plans to implement their PHM strategies under CalAIM, and optimizing PHLC's learning management platform to scale our curriculum across various programs and primary care practices. Reporting to the Director of Programs, the Senior Program Manager will play a key role in shaping, executing, and refining strategies that support California's safety net providers in advancing equitable health outcomes.

The ideal candidate will have experience in designing and implementing large-scale primary care transformation initiatives, leading multi-stakeholder efforts, and leveraging technology such as EHRs and PHM systems to enhance population care. Additionally, the candidate should have expertise in utilizing Quality Improvement (QI) methods to inform program design, execution, and continuous improvement.

Key Responsibilities

Programmatic

• Lead the optimization of PHLC's Learning Management System by collaborating with subject matter experts (SMEs) to develop new courses, testing these courses with end users, and implementing them for PHLC’s programs and services.
• Oversee the design and implementation of new programs, including the development of the technical assistance approach, curriculum framework, and the identification and management of technical assistance partners.
• Foster trusted relationships with clients, technical assistance partners, and participating practices required to ensure delivery of high quality, high impact technical assistance.
• Design, implement, and continuously refine the structure and processes for data collection in both existing and new programs to effectively capture impact and highlight bright spots to better tell the story of our outcomes.
• Collect and analyze driving data to uncover opportunities for ongoing improvements in program delivery, while also providing valuable insights for reporting to funders and key stakeholders.
• Facilitate connections to Medicaid and other statewide programs and initiatives to ensure alignment, optimize resource utilization, scale assets within the LC’s programs, and prevent duplication of efforts.
• Develop clear, compelling communications materials, including high-quality slide decks, newsletters, and other content, to effectively convey key messages and support program goals.
• Support the Chief Executive Officer, Chief Program Officer, and Director of Programs in conceptualizing and designing new program services and products.

Preferred Skills and Qualifications Including:

• 7+ years’ experience in program development, management and delivery in healthcare systems
• Experience in designing and executing large-scale health transformation initiatives
• Knowledge and application of quality improvement science to drive enhancements in processes and outcomes
• Demonstrated experience in population health, health equity, social impact, or purpose-driven work
• High level of emotional intelligence, and facilitation and influence skills
• Exceptional verbal and written communication skills, including slide development skills
• Intellectual curiosity, growth mindset, and tenacity to get the job done
• Strong critical thinking, data analysis, and analytical skills
• Master’s degree in a healthcare related field strongly preferred
• Experience with learning management systems is preferred
• Knowledge of and experience in developing technical assistance strategies to support the adoption of EHR and population health management technologies is preferred

Compensation & Benefits

The salary range for this position is $140,000 - $180,000.
