Summary

Health coaching can potentially address several of the barriers to evidence based care including: limited access to care, poor coordination of care, distrust of and lack of engagement with the healthcare system, missed appointments, inadequate adherence to treatment, and poor patient-provider communication.

The iHealth Certified Health Coach will work closely with a Dietitian and Clinical Assistant in a fast-paced, technology-driven environment to provide patients with high-quality care regarding Hypertension, Hypotension, Diabetes, Pre-Diabetes, and Obesity. This position will be part of a wider-scope endeavor to test the feasibility and potential impacts of a health coach-facilitated and coordinated care plan.

The individual will receive extensive training at the beginning of employment to gain the knowledge and skills needed to work with patients in an individualized manner. The Health Coach will review patient records, gather relevant patient data, meet with referred and enrolled patients, and provide basic health education regarding diet and vitals to support the implementation of the recommendations accepted by the primary care clinician. The Health coach will consult with the Dietitian, Clinical Assistant, and contracted Primary Care Provider regularly, and act as a representative of the care team when following up with patients.

Job Duties

  • Conduct comprehensive health assessments by reviewing vitals (e.g., blood pressure, glucose, weight trends), food diaries, and routine care plan screening tools.
  • Deliver patient-centered coaching virtually and onsite using motivational interviewing and evidence-based practices aligned with iHealth’s Care Standards and national guidelines (AHA, ADA).
  • Support patients in setting and achieving lifestyle and medical goals related to chronic conditions such as Hypertension, Diabetes, Prediabetes, and Obesity.
  • Monitor patient data and trends regularly, providing timely education and interventions to promote clinical stability and improved health outcomes.
  • Address barriers to progress by problem-solving with patients during and between visits, offering guidance and motivational support.
  • Ensure equitable care access by reaching out to patients with limited technology skills via phone or alternative methods.
  • Escalate complex nutrition or medical needs to Registered Dietitians or providers when appropriate, following up on interventions by other clinicians to ensure patient adherence.
  • Mentor junior health coaches through regular audits, feedback, and administrative oversight, including timecards and PTO/reimbursement requests. Caseload adjusted accordingly (75-patient reduction per mentee, up to 5 mentees).
  • Contribute to training, onboarding, and skill development of new team members.
  • Assist with program development and improvement efforts to enhance care delivery and team operations.
  • May create educational content (e.g., handouts, videos, or articles) to support patient learning and engagement.
  • Collaborate with the Clinical Assistant team on patient onboarding, scheduling, data entry, and re-engagement efforts as needed.
  • Uphold ethical standards, build trust with patients and colleagues, and take initiative to resolve issues with a patient-first mindset.
  • Perform other duties as assigned by leadership.

Qualifications

  • Bachelor’s degree in Health Sciences, Nutrition, or a related discipline OR an AA with at least 3 years of experience providing health and wellness counseling, goal-setting, and motivational interviewing
  • Nationally recognized board certification is preferred (NBC-HWC).
  • Language proficiency in Spanish is a plus
  • Organized, proactive, and highly driven to manage time effectively in a fast-paced, dynamic work environment
  • Familiarity with Google suite including drive, calendar, docs, slides, and sheets
