We are currently seeking a Minimum Data Set (MDS) Coordinator to join our team following a valued team member's retirement. This is a highly collaborative role within a supportive, patient-focused team where your attention to detail, regulatory knowledge, and clinical insight will be highly valued. As a MDS, you will be responsible for the following job duties and responsibilities: * Develop, implement and evaluate resident assessment care plans with collaboration of other health professionals to optimize patient well-being. * Assessing and monitoring patient health to meet CMS, MDS/RAI, state and federal compliance requirements. * Oversee the completion of an MDS that is comprehensive, accurate, standardized and reproducible. * Coordinates the completion of Discharge Tracking Forms and Return Tracking forms. * Reviews and distributes Quality Indicator data and other reports to improve MDS accuracy and compliance. * Attends IDT meetings per facility schedule. * Provide educational training related to Medicare, MDS/RAI process to staff as needed. * Performs other duties as assigned.
