Description

The Case Manager's primary responsibility is to educate patients with their chronic disease, including behavioral health diagnosis, through support and oversight. In addition, this position will also provide case management to patients with complex medical/behavioral health illnesses. Nurse Case Manager supports providers and supports staff through an integrated approach to care management and community outreach. Community outreach, such as home visits and health screenings will be required. Essential Functions: 1. Promotes teamwork and enhances a positive work environment. Encourages, supports teamwork, and mutual respect among all coworkers. 2. Possesses excellent customer service skills and acts as a role model and provides exceptional service. 3. Plan, organize, direct, and lead the development of chronic disease program protocols and standards of care. 4. Follow-up with patients via phone calls, home visits and visits to other settings where patients can be found. 5. Maintain library of health promotion and patient education materials and resources. 6. Reviews patient charts and identifies gaps in patient care. 7. Provides on-going supportive and/or case management functions in accordance with the problems, needs, and the strategies identified within the service plan in order to help the patients to achieve the stated goals and objectives. 8. Maintains close communication with the consulting providers, nursing staff, and behavioral health consultants for input regarding medication compliance, side effects of medication, and medication changes, and alerts these members of the staff of any changes in client adjustment which might suggest de compensation and a need for more aggressive intervention. 9. Responsible for developing plan of care, service coordination, and linkages to key community services. 10. Serves as liaison between patients and staff. 11. Completes TB reporting, including medication management for patients who test positive and require public health reporting 12. Patient nurse visits, including TB and Chronic Disease Education. 13. Maintain HIPAA compliance at all times 14. May triage patients by phone and/or walk-ins. 15. Other duties as assigned.

Requirements

Registered Nurse in the State of Nebraska, BSN preferred

Certified - Basic Life Support

3 Years Nursing Experience, Preferred

Excellent problem-solving skills

Computer skills required; Electronic Health Record experience preferred

Highly organized and ability to meet deadlines

Attentive to detail, quality and accuracy

Self-starter and able to work with minimal supervision

Good communication skills
