Summary

Location: McLean, VA Position Type: PRN (As Needed) Position Summary Vinson Hall Retirement Community (VHRC), a premier Continuing Care Retirement Community located in beautiful McLean, VA, is currently seeking an energetic and compassionate Registered Nurse (RN) to join our team in a PRN / On-Call capacity. As an RN, you will work closely with our dynamic Vinson Hall at Home Health team to deliver high-quality skilled home health nursing services to seniors residing within our community. * Current RN license in the state of Virginia (required) * CPR certification (required) * Minimum of 1 year of experience in home health or a healthcare setting * Strong knowledge of OASIS documentation and familiarity with Start of Care (SOC), Resumption of Care (ROC), and Discharge documentation * Excellent communication and interpersonal skills, with the ability to collaborate effectively with physicians, patients, and team members * Ability to prioritize and manage time effectively in a dynamic environment * Compassionate, patient-centered approach to care Primary Responsibilities: * Complete Start of Care (SOC), Resumption of Care (ROC), and Discharge OASIS documentation in accordance with established guidelines and regulations. * Conduct detailed assessments of patients, leading to the development and implementation of personalized Plans of Care. * Provide direct nursing care to patients in their homes, following physician orders and adhering to all healthcare guidelines. * Collaborate with team members to ensure that the Plan of Care is consistently followed and adjusted as needed for optimal patient outcomes. * Communicate effectively with referring physicians, delivering timely and accurate treatment notes, patient progress, and any relevant changes in care. * Prepare for patient discharge by coordinating plans with the patient, care team, and physician as soon as possible to ensure a smooth transition. * Perform other duties as assigned, ensuring a high standard of care and customer service.
