RN Case Manager

Position Summary

RN Case Manager

POSITION DUTIES

Discharge planning - "They are part of the case management team, and have assignments on the inpatient units. Sometimes they are on one unit, most times they have patients on a few units, but in coordination with a provider. "Home base" is the case management department, with possible assignments on Med/Surg, ICU, Tele and Stepdown. We have 4 inpatient units, one ICU. Special Care Nursery and PACU we assist with any needs as well. They do not have any assignments in behavioral health.

Minimum Required Qualifications

Active RN Licensure

Experience in Acute care Setting, plus experience in discharge planning.

Preferred Qualifications

BSN preferred but not required

LENGTH OF ASSIGNMENT

13 Weeks

SHIFT / HOURS PER WEEK

40 hours a week, M-F (8-4:30 or 8:30-5)
