Schedule: Monday - Friday, 8am - 4:30pm

SUMMARY:
SOLAR is a DHS funded Recuperative Care Program serving individuals experiencing homelessness that also have acute medical issues. The RN Care Coordinator will take a leadership role in leading the team of LVNs in the Recuperative Care program and will ensure the quality and adequacy of nursing services provided to members. The RN provides on-site medical management and health education to participants, and liaisons to community and hospital partners within the RNs scope of practice.
Essential Duties and Responsibilities:
* Complete medical exam and assessment at intake to SOLAR.
* Continue treatment plans from the referring entity.
* Provide medical guidance for LVNs in coordinating PCP appointments and/or other medical follow ups upon admission.
* Provide care, and treatment plans to individual members (38 Clients).
* Monitor, reevaluate and manage medical conditions on a routine basis as needed.
* Provide medication management to individuals including but not limited to medication monitoring, patient teaching, and supportive services to increase compliance with medication regimens.
* Provide wound care to members as needed and/ train medical team.
* Develop and revise policies and procedures for the recuperative care program.
* Document and chart all services provided in chart and electronic database with accuracy and in real time.
* Promote patients' independence by establishing patient care goals, teaching patients to understand their medical condition, medications, and support self-care skills.
* Work in collaboration with the rest of the SOLAR team- case managers, and mental health provider, as appropriate, of any medical and/or medication concerns.
* Maintain medication logs and medication storage in compliance with safety standards and industry standards of care.
* Provide crisis intervention assistance when needed.
* Be able to notify the appropriate community agency in an emergency (i.e., police, paramedics, fire department).
* Maintain open communication with direct supervisor, individual and group supervisor(s).
* Cooperatively and cohesively work with other parties involved in the member's case.
* Participate in staff meetings, nursing meetings, and supervision.
* Ensure that medical services are reflective of the agency's treatment model, which emphasizes member choice, harm reduction, trauma-informed care, and psycho-social rehabilitation.
* Conduct Health/Wellness and medication management groups.
* Maintain confidentiality of member files per HIPAA and all applicable guidelines
Qualifications:
* Experience as a RN, two years' preferred.
* Knowledge of and practice in the methods of harm reduction
* Experience working with individuals with mental health and substance use issues.
* Current valid California Driver's License
* RN Credentials
Preferred Qualifications:
* Experience working with those living with mental illness and/or substance addictions.
* Experience completing DHS documentation.
* Bilingual Spanish preferred.
* Minimum 35 WPM typing speed preferred.
Work Environment:
* Office environment with occasional field activities with SOLAR participants and staff.
* Regularly required to sit, stand, bend, and occasionally lift or carry up to 35 pounds.
* May be exposed to elements like cold, heat, dust, noise, and odor.
* May need to bend, stoop, twist, and sit throughout the day.
