Responsibilities

This position will ensure completion and accuracy of the RAI. Contribute to the care planning process, participate in quality improvement and PRI activities and Medicare Part A tracking.

Job Responsibilities & Standards

  • Review patient records for doctor’s notes, therapy notes, hospital records, interdisciplinary assessments, flow sheets, and care plan.
  • Collect data for appropriate time frames / complete the MDS.
  • Clarify documentation as needed with appropriate disciplines.
  • Review patient care plans to make sure triggered areas are addressed in the patient care plan as appropriate.
  • Communicate any discrepancies in documentation to the Nurse Manager, Assistant Nurse Manager, or appropriate discipline.
  • Communicate with interdisciplinary team and MDS Technical Office when a patient appears to be a significant change.
  • Perform ROM and Pain evaluations per facility policy and report declines or concerns to the Nurse Manager or Charge Nurse.
  • Fill out re-certification forms for Physician signature and track re-certifications to ensure they are being completed in the designated time frames per regulation.
  • Notify MDS Technical Office of those patients who no longer meet the Medicare Part A criteria and monitor for 30 days those patients who have been cut, to see if they become Medicare Part A eligible during that 30-day period. Notify the office when a patient resumes eligibility in order to initiate new MDS completion schedule.
  • Complete and submit for signature a re-certification form on any patients who qualify for Medicare Part A coverage to be resumed, under regulatory specifications.
  • PRI completion for transfers, TBI waiver, and NH waiver programs, etc.
  • Responsible for employee injury prevention, follows safety rules, and addresses safety issues with supervisor.
  • All other duties as assigned by supervisor.

Qualifications

  • Current New York State Registered Nurse License and 3 years of RN experience
  • Strong verbal, written, and computer skills required.
