Behavioral Health Care Manager – Make an Impact from Day One!

As a Behavioral Health Care Manager, you’ll be on the front lines of care, using your clinical expertise, communication skills, and compassion to support patients navigating behavioral health and chronic conditions. What You’ll Do
Be the frontline – Reach out to patients by phone (expect 60+ daily calls) and provide meaningful behavioral health support.
Guide with purpose – Use tools like PHQ-9 and GAD-7 to conduct screenings and develop care plans.
Coach for change – Apply motivational interviewing to help patients set and achieve SMART health goals.
Coordinate and connect – Work closely with primary care providers, behavioral health consultants, and other healthcare team members.
Stay organized – Document all interactions, update care plans, and track outcomes in the EHR.
What You Bring
LPN/LVN or Certified Medical Assistant (MA) – Required
(Compact license is a plus, but not required)
Experience in behavioral health, care coordination, or chronic disease management – Preferred
Excellent communication skills and a natural ability to build trust with patients
Strong organizational habits and comfort working in high-volume, metric-driven roles
Proficiency with electronic health records (EHRs)
Bilingual in English & Spanish is a plus!
Schedule
Full-Time | Monday–Friday
Choose your shift: 8:00 AM – 5:00 PM or 9:00 AM – 6:00 PM (time zones do vary) main hours reflect Pacific time zone.
Take a 30-minute or 1-hour lunch—your choice

Patient Reach: 21–22 patient connections per day, 60+ calls daily
Quality Documentation: Timely, complete, and accurate EHR entries
Care Impact: Measurable improvements in PHQ-9 and GAD-7 scores
