Position Summary
The Enhanced Care Management (ECM) Regional Manager plays a pivotal role in guiding and supporting an assigned care team, ensuring the provision of high-quality care to members. Collaborating closely with senior leadership, the ECM Regional Manager empowers the team with tools, training, and resources necessary for success while upholding the standards of excellence in care management. The ECM Regional Manager will manage, assign caseloads, and provide day-to-day program coordination for the ECM Program with the support and full knowledge of the ECM Director. The ECM Regional Manager duties encompass a wide range of responsibilities, from leadership and support to direct member engagement, all with the overarching goal of enhancing the quality of care and support provided to members.
WHERE YOU’LL WORK
This position is hybrid; 30% of duties performed remotely and 70% traveling to in-person member visits in assigned regions (San Francisco, Alameda, Contra Costa, Santa Clara, Sacramento, Salano counties, etc.). Standard business hours are Monday through Friday from 8:30 am to 5:00 pm. After hours may be required to meet business demands.
What You’ll Do
Oversight
- Oversee a team of supervisors and other staff across the Northern California region
- Own the quality metrics and develop improvement plans given MCP audit tools with expectation of superior audit results
- Coordinate team meetings and accurately address team questions and concerns
- Manage the outreach, engagement, enrollment, intake, assessment and tracking of new enrollments for the ECM Program, as well as reassessment and reauthorizations for continuing members
- Assist with file compliance and audit preparation in coordination with the ECM Director
- Ensure all charting is compliant with Department of Health Care Services (DHCS) standards at all times
- Report all non-compliance to the ECM Director for corrective actions
- Data collection and reporting
Service & Support
- Assign caseloads to team members
- Facilitate and link members to appropriate MCP resources
- Manage and conduct regular team meetings with staff to review progress towards goals
- Reassess and revise service plans using recovery-oriented service models
- Facilitate and lead consistent in-person team meetings
- Manage staff and ensure educational and employment goals are being met during scheduled meetings
Administration
- Build and maintain market growth strategy
- Meet regularly with assigned ECM Director & health plan to identify areas for membership growth and outreach improvement
- Conduct financial analysis in collaboration with the Finance team on assigned Managed Care Plan (MCP) to ensure positive profit and loss for the unit
- Act as key leader for assigned health plans
- Build and lead effective service delivery team measured by staff retention and MCP growth
- Establish and maintain effective customer relationships
Documentation & Data Collection
- Work collaboratively with members of assigned ECM teams as needed
- Provide support and effectively perform assigned job duties
- Contribute to the achievement of the ECM program's goals and objectives of housing, income stability, healthcare goals, sobriety, and behavioral health goals
- Attend staff meetings, supervision, and training as required
- Attend community meetings as requested
- Perform other duties and projects assigned
Who You Are
- Fluent in English (written and verbal)
- Operate a computer and other office equipment such as a telephone, calculator, copy machine, and printer
- Must be able to remain in a stationary position 30% of the time.
- Must be able to move around and visit members 70% of the time.
- Frequently moving or carrying office equipment weighing up to 15 pounds
- Ability to operate a vehicle and/or travel to meetings at different locations as required or requested
What You’ll Need
- High school diploma or GED required
- Bachelor’s degree in healthcare or related field, or 4 years of healthcare experience in lieu of education
- Minimum 5 years of care management experience with at least 2 years of supervisory or managerial experience
- Current and valid Driver’s License
- Proof of auto insurance
- Distraction-free home workspace with a secure internet connection

- Bilingual in Spanish
