Job Summary
This position is responsible for organizing projects involving health and housing integration, and will drive quality improvement, analytics, and metrics in the development of this work. This position works closely with the Integration, Quality and Analytic Insights teams and will focus on the buildout of new clinical programs which improve the coordination between homeless services provider, county housing systems, health plans, community-based organizations and the CCO. This position will additionally coordinate with new Health Related Social Needs benefits operations covered under the Oregon Health Plan. This position will assist with the development of data tools including analytic reports and data dashboards to support the work of internal and external partners. Essential Responsibilities
Areas of Focus
- Healthcare and housing integration including:
- Centralization of comprehensive project wide data collection
- Tracking of contract deliverables
- Participation in regional workgroups
- Quality improvement initiatives, both within the organization and engaging with external partners.
- Tracking major projects including the CCO’s Cross Sector Case Conferencing and Regional Integration Continuum processes.
- Analytic projects including comprehensive data approaches for key areas (e.g., social determinants of health).
- Supporting dashboard and analytic tool development priorities to enable data-driven organizational strategies.
Planning and Development
- Develop project plans that outline components, communications, milestones, priorities, needs and resources.
- Organize, facilitate, and participate in meetings of stakeholder groups to identify needs and build consensus and collaboration.
- Work collaboratively in analyzing business needs to identify potential technical solutions and process impacts to inform business case, project proposals and project plans.
- Partner with analysts and team members to define and document business requirements, IT requirements, and workflows (current and future state).
Project Management
- Apply senior level skills in leading stakeholders; act as a resource to project team members.
- Utilize project management methods and tools to communicate project goals and resource requirements and obtain stakeholder sponsorship.
- Utilize project management and resource tracking tools (including Jira) to monitor and regularly communicate project status to sponsors, business owners and other stakeholders, helping to ensure progress toward completion.
- Make formal and informal presentations and reports, providing updates and other relevant information on the status of projects.
- Represent the organization regarding project status and alignment with strategic plans; facilitate and work effectively to develop consensus within divergent groups and/or groups with divergent viewpoints.
- Identify potential difficulties and/or barriers to project goals and take steps to mitigate or avoid them.
- Ensure compliance with all applicable standards, policies, laws, rules, and regulations outlined in the project plan.
- Coordinate the project work of contracted partners including overall management of analytic and evaluation deliverables throughout the development process to final product.
Implementation
- Employ advanced change management techniques to facilitate the adoption of the project.
- Track large scale project costs and identify variances from project plans.
- Develop and recommend operational processes and/or process improvements including supporting information systems.
Documentation and Evaluation
- Maintain appropriate records, information, documentation, and reports.
- At the time of project completion, close files, and review records to ensure documentation is appropriate and complete.
- Facilitate the evaluation of progress and challenges following implementation, identifying potential system or process improvement opportunities.
- Provide subject matter expertise and guidance for implementation, configuration and on-going support of internal systems, technology, and analytics.
- Coordinate alignment of systems, processes, and internal and external resources
- Recommend solutions to leadership to resolve schedule, budget, and resource conflicts.

Knowledge
Knowledge, Skills and Abilities Required
- Knowledge of principles, methods, and techniques of healthcare policies, practices, and technologies or
- Knowledge of housing/ homelessness policies, practices or technologies
- Knowledge of project management methodologies, including Waterfall / Classic, Agile, Scrum, and project communications
- Knowledge of SharePoint
- Functional and analytical knowledge of HIPAA compliant transactions

Required
Education and/or Experience
- Minimum 5 years’ increasingly responsible experience in project management including a minimum of 2 years project management experience with large, complex projects
Preferred
- Minimum 5 years’ experience in Scrum, Agile, and/or Project Management International (PMI) Project Management certification
- Minimum 3 years’ experience in housing/ homelessness, health care or health care administrative setting
- Experience with quality metrics
