Imagine! has an exciting opportunity to connect individuals in the intellectually and developmentally disabled community to vital services and resources.
Purpose of Position- Leads the development, outreach, and intake processes within the Community Partners team, focusing on connecting individuals with disabilities, mental health challenges, and other barriers to essential services. Establishes and maintains strong relationships with local agencies and ensures that residents are effectively referred to and connected with the appropriate supports. Oversees the outreach efforts of the Community Partners team, drives strategic initiatives, identifies emerging needs and trends, and ensures that the Community Partners team remains responsive to the evolving landscape of client care.
Essential Duties/Responsibilities
* Strategic development and outreach
* Leads the development and execution of outreach strategies to engage community partners, stakeholders and residents.
* Ensures the Community Partner team effectively reaches and serves the target populations.
* Intake and referral management
* Oversees the intake process for individuals serviced in or by community organizations (such as Boulder Housing Partners, All Roads, Mental Health Partners).
* Ensures that residents are accurately assessed and referred to the appropriate services.
* Works closely with community organizations to streamline referral processes and improve client access to care.
* Community-based advocacy
* Acts as primary advocate for residents, representing their needs in the community.
* Collaborates with local, state, and national agencies to enhance service delivery and address systemic barriers.
* Partnership development
* Builds and maintains strong relationships with community organizations.
* Represents Imagine! on councils, committees, and task forces to advance the organization's mission.
* Program and service alignment
* Ensures that the Community Partners activities align with Imagine!'s overall mission and goals.
* Works with Assistant Director to identify and address gaps in service delivery.
* Residential Program Management
* Ensures regulatory compliance as well as compliance with company policy and procedure for residential services offered through Community Partners, including contracted and independent living services.
* Supports the Assistant Director to develop, implement, and monitor departmental budget. Creates and implements action plans to address budget variances, under the supervision of the Assistant Director.
* As a member of the Residential Management team, regularly seeks to develop and implement programs, processes, and quality improvement initiatives.
* Responds to emergency situations on a 24-hour basis, working with case managers, site supervisors, etc.
* Team leadership
* Supervises and provides leadership and support to the Community Partners team.
* Fosters collaborative and supportive environment, and promotes professional growth and high-quality service delivery.
* Ensures effective communication across the Community Partners team and with other departments.
* Disseminates information and gathers feedback to inform decision making.
* This position requires regular use of a personal vehicle with liability insurance that meets Imagine! standards.
Job Qualifications
Knowledge, Skill, and Ability:
* Extensive knowledge of intellectual/developmental disabilities (IDD), mental health, and other public assistance programs in Boulder County and Colorado.
* Knowledge of service coordination and referrals processes within a complex care environment.
* Strong leadership and supervisory skills.
* Effective written and verbal communication skills with individuals and groups at all professional levels.
* Ability to engage and influence a wide range of stakeholders.
* Ability to build and maintain strong community partnerships while advocating for client needs.
* Demonstrated organizational and time management skills.
* Ability to work independently and prioritize tasks/goals for self.
* Ability to work effectively with various technologies including MS 365, internet-based databases, Excel, Outlook, Word, etc.
* Effective problem solver.
* Ability to handle sensitive information and maintain confidentiality.
* Possession of a valid drivers' license and ability to meet Imagine! driving requirements.
* This position requires use of a personal mobile device such as a smartphone or tablet.
Training/Education:
* Bachelor's degree in human services, social services, psychology, public health, or a related degree preferred.
Experience:
* Two years' experience in a leadership role with an organization serving people with IDD or similar required.
* Two years' experience in community outreach, advocacy, or related field required.
Working Environment/Physical Activities
* Effective with shifting roles, responsibilities, and expectations in a changing environment.
* Ability to work effectively under the pressure of deadlines.
* Regular travel within the Boulder/Denver area.
