Our committed staff of direct support professionals help drive our mission. What We're Looking For: The Rock Creek Foundation is looking for a Full Time Program Manager of Community Outreach. The Program Manager of Community Outreach provides integrated goal-oriented individual and group activities, while coordinating opportunities for interdepartmental synergy and a broad-based continuum of training and learning opportunities for the individuals we serve and all staff for consistency. Ensures the completion of self-determined goals including: assistance in job seeking skills and the development of employment opportunities. Encourages and documents individuals goals which address activities of daily living, budgeting, developing natural supports within the community, behavioral management and participation in groups and activities. The ability to effectively deescalate crisis situations. Further duties include completion of assigned paperwork. What You'll Do: * Being a key player in various programs that are 100% community based by mentoring and training in the integration of interdepartmental staff activities and building and modeling interdepartmental duties * Help train and maintain the environment of collaboration of staff from different departments, to have a larger understanding of where a person works, lives, or socialize * Assesses individual's behavioral, social, emotional, financial, health, vocational, or other problems to help develop rehabilitation treatment plan, or Individualized Plan goals * Assists staff in planning and adapting materials and activities to enhance the individual's day; ensures consumer's safety and to ensure least restrictive environment and maximum participation * Facilitates program department staff meetings; provides information regarding consumer, attends annual PCP meetings * Depending on assignment, monitors, corrects, and instructs consumer's behavior, de-escalates anger and frustration, assists consumer in conflict resolution from harming self or others; keeps consumer attention focused on activities or work duties * Maintains communication with residential administration staff, house staff, Personal Support staff and Community Development Services staff (to include 1:1's) usually through meeting with the individual and staff member (if applicable) one time per week; written progress notes and assist with maintaining consumer's chart, scheduled activities and/or community based meetings and activities. * Models appropriate behavior for consumer; demonstrates and discusses behavior, reactions, and feelings to assist consumer in proper work and/or social behavior. * Depending on assignment may supervise consumer at a job site on Rock Creek contracts or outside premises; instructs and encourages consumer * Depending on assignment may carry a caseload and/or assist in ensuring daily staffing needs, ratios are met in Personal Supports, Community Development Services, Residential and/or Day Habilitation Program. * Assisting individuals to identify and resolve problems and make effective use of resources. * Prepares and maintains electron or written daily, monthly and whenever appropriate case records and reports. Performs case follow-up and closing, and performs other administrative tasks as required. * Organizes and conducts support group and vocational groups as needed with MHVP individuals as well as Day Habilitation individuals. * Assists individuals (MHVP/DD) in obtaining paid employments and/or volunteer work, depending on scope of program and funding source * Observes worker to ensure that work is performed according to prescribed methods and meets established standards * Maintains working relationships with staff of other agencies and institutions, homes, facilities, and acts as liaison between clients and agency/ institution * Helps clients understand mental illness and how it affects their health, life styles, and employment. * Reviews evaluation made on individual to ascertain type of work recommended and limitations due to specific disability * Prepares and maintains written case records, reports, performs case follow-up and closing, and performs other administrative tasks as required * Review of staff contact notes, annual assessments, monthly progress notes and LTSS (State billing system) to ensure accurate billing * Counsels clients, assisting them to identify and resolve problems and make effective use of resources * Encourages clients to do things for themselves and obtain employment to retain feelings of independence and self-esteem * Attends mandatory meetings * Attends all regional and State informational meetings to ensure that the programs are in compliance with State Waiver Requirements * Responsible for screening intakes for program growth * Checks work email at the beginning and end of each shift * Participates in and successfully completes all required trainings. * Transport individuals in personal vehicle * Other duties as assigned What We Require: * Required to possess a valid driver's license. * Bachelor's degree from four-year college or university OR three (3) to four (4) years related experience and/or training OR an equivalent combination of education and experience. * Knowledge of Internet and Microsoft Office software. * Must be able to work with "high-risk" consumers who have mental illnesses as well as developmental and physical disabilities.
