Set to open in June 2025, Estuary I provides 44 units of Permanent Supportive Housing (PSH) in Alameda. Chronically homeless disabled residents with complex needs will receive housing and support services including on-site engagement, case management, mental health and linkages to healthcare, and community building activities. The property is owned by the Alameda Housing Authority, with services provided by Building Futures.

POSITION SUMMARY: The Program Manager/Case Manager will play a pivotal role in the successful operation of Estuary I, a 44-unit Permanent Supportive Housing (PSH) program in Alameda, CA. This position is responsible for overseeing the delivery of comprehensive support services to chronically homeless disabled residents, ensuring they receive the necessary resources to build safe and stable lives. This position is also responsible for assessing client needs, developing personalized case plans, and facilitating access to community resources. They will work collaboratively with clients to empower them toward sustainable housing solutions and improved well-being. This is a full-time on-site position and will be primarily located at Estuary I site at 510 Mosely Avenue, Alameda. The Program Manager/Case Manager will supervise 2-4 positions, oversee the program, and serve as primary contact for property management.

ESSENTIAL DUTIES:

  • Oversee the Estuary I Permanent Supportive Housing Program, ensuring that objectives are met within the timeframe established.
  • Provide oversight of all intakes and individualized needs assessments for program participants.
  • Oversee participant-related data tracking systems, including case notes and database.
  • Support the client in their progress/regress and keep them focused on meeting their goals, focused on client strengths.
  • Assist residents in retaining housing and maximizing their independence and self-sufficiency by providing linkages to appropriate community services such as crisis intervention, rehab, income support and benefits acquisition, employment assistance, primary physical and mental health, substance recovery and family involvement.
  • Provide crisis intervention as needed and when requested by property management and/or program participants.
  • Collaborate with property management in creating an environment that fosters a sense of ownership for program participants and builds community.
  • Develop collaborative relationships with other local service providers and community agencies. Maintain a positive relationship with the surrounding neighborhood.
  • Develop creative ways to engage clients; keep records of the effectiveness of each success.
  • Develop a risk management plan and crisis planning tool for staff to implement.
  • Prepare and gather statistics and information for grants and board meetings.
  • Oversee resident programs and, with relevant staff and volunteers, continue to develop innovative approaches to meeting the needs of program participants.

QUALIFICATIONS & COMPETENCIES:

  • Master’s degree in social work or equivalent with a minimum of two (2) years post graduate experience.
  • May be replaced by bachelor’s degree in social work or equivalent with a minimum of four (4) years post college experience.
  • Two years of management/supervisor experience.
  • Administration experience in case management and program planning; preference given to those with experience working with homeless populations.
  • Knowledge of the HMIS or other homeless population tracking systems.
  • Knowledge of resources for homeless and other underserved populations in Alameda County.
  • Supports the goals, philosophy and values of Building Futures with Women and Children.
  • Maintains professional standards of confidentiality.
  • Represents Building Futures at meetings with property management, funders, and other partners.
  • Participates in on call staff rotations.
  • Completes referrals for residents applying to outside agencies for housing and assistance.
  • Ability to interact with staff, residents, volunteers, and community professionals in a professional, positive manner.
  • Excellent critical thinking skills and analytical ability.

  • Sufficient manual dexterity to enable the employee to operate a personal computer, phone, and related equipment.
  • Must be able to lift 25 pounds.
