Position Summary:
The Clinical Program Coordinator has a primary role in the oversight, coordination, and implementation of multiple clinic-based programs at Roots Community Health Center. Assigned programs may vary based on organizational priorities and funding; examples include HIV Prevention and Management; Substance Use Disorder Treatment for opioid and stimulant use, Hepatitis C detection & treatment; Diabetes Self-Management Education & Support, Sexually Transmitted Infection (STI) detection and treatment, COVID, and Complex Care. This position is based at the Main Adult Clinic with travel to other Roots sites, as needed.
Duties and Responsibilities:
Assist in tracking all program/contract deliverables and ensure they are carried out in a timely and effective manner.
Meet and communicate with individuals at all levels of the organization, community partners, and funders to advance program goals and raise/maintain program awareness.
Coordinate activities of the navigation, outreach, and patient services staff.
Ensure consistent and complete documentation of program services and activities.
Report data collection problems to the Program Administrator and Director in a timely manner.
Keep internal staff, particularly clinic and navigation, up to date on program offerings, requirements, and linkage points; identify and troubleshoot gaps in communication or the continuum of care.
Oversee patient recruitment, enrollment, and outreach efforts to maximize participation in programs.
Identify gaps in training and areas for program improvement and address or elevate to management.
Attend all relevant meetings, and contribute to note-taking, reporting, and advocacy efforts.
Draft and implement protocols, training materials, and communication materials.
Contribute to program innovation and clinical strategy development.
Participate in staff meetings, agency meetings and staff training.
Competencies:
Interest and willingness to learn about basic disease management concepts and health messaging specific to the programs.
2 years of experience working in a non‐profit organization, or a community clinic preferred.
Excellent communication skills (written and oral) with comfort in presenting training and motivating change among various stakeholders.
Ability to discuss stigmatized health conditions (Hep C, HIV, opioid dependence) and behaviors (needle sharing, unprotected sex) in a nonjudgmental manner.
Strong organizational skills.
Proficient in computer use, including MS Office and G-Suite, and ability to learn new Electronic Medical Records (EMR) and proprietary database systems.
Interest in working with low-income individuals, especially people of African descent, in an effort to empower the community and eliminate disparities.
