Program: Supported Community Connections (SCC)
Position: Direct Support Professional
Reports to: Director
General Description: Provide opportunities for persons with intellectual and developmental disabilities (IDD) to access their community for socialization and skill development. Support persons with companionship, mentoring, and achievement of personal goals.
Qualifications:
· Active and enjoy planning and participating in a variety of community activities.
· Be at least 18 years of age.
· Be qualified by education, knowledge and experience to oversee the services provided.
Valid Driver’s License, good driving record, and reliable personal transportation.
Resident of the State of Colorado.
Computer and smart phone skills.
Ability to pass criminal/adult protective services’ background checks.
Performance Outputs
The responsibilities may include the following:
Provide direct care and supervision.
Assist individuals in accessing the community.
Life skills maintenance and acquisition.
Medication administration.
Support with overall health and wellbeing.
Daily program documentation.
Compliance with all policies and procedures.
Transportation support.
Plan and schedule community activities.
Assistance with activities of daily living and personal hygiene.
Provide support for medical protocols, such as seizure, elopement, special diet, etc.
Assistance with behavioral challenges.

· Manage and coordinate daily schedule.

· Complete Orientation

Job Types: Part-time, Contract
Pay: $18.25 - $20.00 per hour
Expected hours: No more than 25 per week
