Job Responsibilities

• Develop and implement individualized care plans
• Coordinate and oversee the care of patients
• Provide hands-on nursing care as needed
• Collaborate with interdisciplinary teams to ensure the best possible outcomes
• Act as an advocate for patients and their families
• Assess patient needs and provide appropriate interventions
  
  

Qualifications

• Active RN license in the state of Georgia
• Minimum of 2 years of nursing experience
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Knowledge of hospice and palliative care preferred
