Position Objective
The Manager of Quality & Population Health is responsible for overseeing and providing leadership support to the Alexandria Clinic Quality & Population Health and Health Care Home teams. This position oversees team processes to ensure Alexandria Clinic is meeting quality and population health standards set forth by payors, federal, and state guidelines. Using data analysis, the Manager of Quality & Population Health tracks various metrics progress and applies insights to address gaps in care, introduce improvement initiatives, and create population health programs that are focused on enhancing patient outcomes, enriching the patient experiences, and reducing the total cost of care.
The Manager of Quality & Population Health collaborates with clinical teams, organization leadership, and community partners on quality improvement and population health initiatives.
Essential Responsibilities
Quality and Population Health Delivery
Develop and collaborate with the interdisciplinary team to implement quality and population health initiatives
Use data analysis to track progress and apply best practices to address gaps in care
Monitor performance, deliver feedback, and use data to inform process changes aimed at improving efficiency
Maintain knowledge of the various quality measure and population health metrics and communicate changes to affected clinical staff and leadership
Regularly communicate with nursing staff, physicians, providers, senior leadership, and organizational leadership
Build and maintain strong relationships with internal teams and community partners to facilitate population health and care management initiatives
Other Responsibilities
Perform all other related duties as assigned in a professional and responsive manner
Demonstrate the ability to use relevant equipment
Support, understand, and promote Alomere Health’s mission, vision, values, policies and procedures
Ability to meet the work schedule requirements with flexibility dependent upon the needs of the department

Required Qualifications
Associate’s Degree in Nursing
Minimum of three years as a Registered Nurse and one year in a supervisory position
Must be a Registered Nurse in the State of Minnesota
CPR/BLS Certified
Preferred Qualifications
Bachelor’s Degree in School of Nursing
Knowledge of and experience with Medicare Advantage, Medicare Shared Savings Programs, Accountable Care Organizations, Health Care Home, care coordination, and transitional care management
Knowledge of care management, population health, and quality measures.
Knowledge, Skills and Abilities
Skilled in Microsoft Office and general computer knowledge
Effective communication and presentation skills
Leadership
Demonstrate knowledge and application of established practices, protocols, and procedures of the position, department, and applicable professional standards
Maintain competency and knowledge of current standards of practice, trends, and developments in related scope of job role or practice; commits to continuous learning for themselves
Assist in development and fostering of continuous learning for members of team
Promote a culture of safety and strong communication for patients and employees
Demonstrate excellent customer service through attitude and actions, consistent with the standards contained in the Mission, Vision, and Values of the organization
