🧭 Your Role:

• Bring primary care to the patient—where they live and feel most comfortable.
• Diagnose, manage chronic conditions, and create personalized treatment plans.
• Build genuine connections with seniors and their families.
• Collaborate with our care team to deliver holistic, heart-centered care.
  
  

📋 What You Bring:

• A current and unrestricted NP or PA license in California.(New Grads Welcome)
• Experience with geriatrics, home-based care, or primary care a huge plus.
• A love for serving seniors and a desire to make healthcare more human.
