We are a non-profit health center that provides primary care medical and dental services to all regardless of insurance status. We accept patients with or without insurance and provide health care discounts based on income and family size using the federal poverty guidelines.

About CHDC

CHDC (Community Health and Dental Care, Inc .) is a quickly growing Community Health Center in Pennsylvania with locations in Pottstown, Barto, and Boyertown, providing care for all patients in our service area, regardless of insurance status or ability to pay.

Our staff put the care and well-being of our patients at the forefront of everything we do at CHDC.

Our History: Community Health and Dental Care was formed through the generous funding of the Pottstown Area Health and Wellness Foundation along with various local businesses. Our doors opened in October 2008 with 39 medical and dental patients seen that first week and we’ve grown to see thousands of medical and dental patients each year. As a non-profit community health center, we have been organized to become the patient’s medical home for preventive care and to promote overall well-being, thus reducing the number of emergency room visits.

About The Patient Services Representative Position

Provides general clerical support to primary care teams and general customer service for health center patients and visitors.Create an atmosphere that is receptive to other’s thoughts and concerns to provide compassionate care between our patients and effective communication with the staff.

Duties Include But Are Not Limited To

  • Performs duties of Front Desk Secretary/Receptionist as needed (greeting patients and visitors, patient intake and input of all required registration information, scheduling appointments, etc.) at assigned area within the health center.
  • Must be able to work schedule assigned by supervisors, including evenings and Saturdays if needed.
  • Uses check-in software efficiently.
  • Verifies and updates patient information at each visit (address, phone, insurance, etc.)
  • Intake of insurance information and verification
  • Management of patient flow
  • Checks patients out at the end of appointment including giving follow-up appointments as require for all departments.
  • Provides access to the patient portal; instructs and assists (when needed) the patient to enroll.
  • Supports Billing Department by documenting correct billing information at the point of service (including eligibility verification, updating insurance information, and appointment tracking)
  • Collects co-payments and payments from patients at the point of service. Also making sure that your cash drawer balances daily.
  • Provides patient services, including resolving complaints, making daily reminder calls, etc.
  • Assist with answering calls, routes call, takes patient messages or returns calls in a timely manner, sets appointments according to protocol.
  • Checks PSR mailbox and NextGen patient portal daily.
  • Provides translation services as able and appropriate.
  • Performs all assigned duties in a manner consistent with maintaining a safe and respectful patient care environment.
  • Attends department meetings and all mandatory health center meetings.
  • Participates in Care Team meetings.
  • Responsible for Care Team Provider panel.
  • Completes department reports as assigned.
  • Performs other related duties as required or assigned.

Requirements

Qualifications, Education, and Knowledge

  • High school diploma or equivalent
  • Medical or dental office experience or background required; customer service experience preferred.
  • Attendance required at assigned CHDC site(s)
