A Brief Overview

The Patient Care Coordinator Lead assists with the administration aspects of day-to-day patient care coordination team and operations of the Physical and Sports Medicine Department. In addition to their administrative responsibilities, this role is also responsible for all duties and functions of the patient care coordinator role for the Physical and Sports Medicine department.

What You Will Do

  • Performs proficiently in the regular duties of a Patient Care Coordinator - scheduling services, including scheduling/rescheduling of new and returning patients. Verifies insurance information and registers the patient. Other office duties include performing updates in Epic, coordinating patient classes, managing referrals, managing patient account and billing work ques.
  • Assists with PCC staffing, scheduling, and PTO requests.
  • Assist in ensuring standard work assignments are followed by the PCC team.
  • Attends monthly front desk leadership meetings.
  • Compiles and reports daily snapboard data & department weekly/monthly metrics.
  • Assist with patient escalations & service recovery.
  • Assists with onboarding and training of new employees.
  • Greets patients and manages phone calls
  • Provides information on various department clinical programs to patients during patient scheduling/intake call.
  • Schedules new and returning patient appointments.
  • Coordinates virtual and in-person classes and support groups
  • Manages referral, patient and billing workques.
  • Sends communication to referring physician offices when appropriate.
  • Scans patient related documents for various clinical programs.
  • Requests relevant laboratory or imaging data in preparation for patient visits.
  • Builds customer relations through positive interaction with patients and other hospital departments.
  • Consistently and accurately registers patients in Epic with all required information and keeps up with new registration requirements.
  • Responsible for obtaining insurance and/or employer authorization as applicable.
  • Responsible for explaining benefits and co-payments to patients as applicable.
  • Responsible for subpoena and medical record requests, locating medical reports in various systems.
  • Ensures the daily review of email, messaging & EPIC systems for department communications.
  • Participates in department in-services.
  • Participates in and assists in leading process improvement projects.
  • Assists in developing and implementing new workflows for the PCC team.
  • Performs annual education modules and HealthStream education requirements on time.
  • Assists with inventory and replacement of supplies by ordering or notifying supervisor.
  • Attends huddles and mandatory in-service meetings.
  • Ability to work flexible schedules and varying locations, as assigned.
  • Accepts assignment of other duties as needed, receives training and/or orientation.
  • Employee must perform all duties and responsibilities in accordance with the C-I-CARE Standards of the Hospital. C-I-CARE is the foundation of Stanford Healthcare – Tri-Valley patient-experience and represents a framework for patient-centered interactions

Education Qualifications

  • Associate's Degree Preferred

Experience Qualifications

  • 3+ years to 5 years medical office experience Required

  • Excellent communication skills.

  • Working knowledge of medical terminology.
  • Working knowledge of insurance terminology.
  • Strong organization skills and attention to detail.

Base Pay Scale: Generally starting at $25.38 - $33.50 per hour

The salary of the finalist selected for this role will be set based on a variety of factors, including but not limited to, internal equity, experience, education, specialty and training. This pay scale is not a promise of a particular wage.
