Summary

Ignite Community Network is passionate about supporting people with a disability (Individuals) to increase their independence, capacities and opportunities to live a full and active life. As the NDIS Service Coordinator you will work with Individuals to develop and facilitate services to assist them achieve identified goals. You will work within a family network which comprises of family, friends, community groups, social clubs and other people who are identified by the Individual. The Service Network Coordinator will identify where, when and how supports will be provided, as well as the team that will facilitate services.

MAIN DUTIES/RESPONSIBILITIES

• Assist with managing agreements that takes into account goals, strengths, challenges and supporters (Support Network)

• Develop new service plans

• Provide reports on progress of program activities including recommendations

• Provide specialist expertise or advise with regard to Programs and Services

• Assist in the development of programs and procedures

• Assist to manage funding allocations for NDIS participants

• Review person-centred plans and agreements.

• Monitor and evaluate services provided to Individuals.

• Complete progress notes, record incidents and feedback

• Supervise Disability Support Workers including informal and formal meetings, reviews and associated reports.

• Participate in on-call roster including weekends

• Communicate rostering changes to Staff

• All necessary paperwork in relation to Services is consistently up to date

SELECTION CRITERIA

Essential

1. Tertiary qualification / or other relevant qualification or experience in disability sector, mental health, aged care

2. Experience working collaboratively with multiple stakeholders in formal and informal partnerships to achieve outcomes.

3. High standard and ability to articulate information, factual and evidence based written reports.

4. Experience in supervision of staff.

5. Experience in scheduling services and/or staff.

6. Strong interpersonal and communication skills.

7. High level of computer proficiency in MS Office suite.

Desirable

1. Knowledge of the National Disability Insurance Scheme (NDIS) and the disability sector.

2. Experience using Datanova FlowLogic software.

3. Experience using Microsoft Suite
