Purpose

Job Description

The purpose of this position is to improve the overall quality, completeness, accuracy, specificity and timeliness of physician clinical record documentation. To optimize CRG reimbursement and Risk Scores to facilitate data quality in the hospitals Outpatient Services.

Specific Duties And Responsibilities

  • Complete retrospective reviews of patient records to evaluate documentation to assign the principal diagnosis, pertinent secondary diagnoses, post admission complications and procedures for accurate Coding and Billing.
  • Queries physician for incomplete, inconsistent, unclear or conflicting documentation to clarify and resolve conflicting information in the medical record prior to discharge and or billing.
  • Maintains a record of review and query activities and other appropriate records.
  • Communicate with clinical staff on clarification of clinical criteria needed to support documentation in the record.
  • Assign ICD-10 Diagnostic Codes as well as CPT Procedural Codes and E&M Leveling
  • Attends and/or participates in staff departmental and interdisciplinary meetings, process improvement events, training and quality assurance/performance improvement activities.
  • Performs other related work, as assigned by supervisor.

Minimum Qualifications

  • High school diploma or its educational equivalent, approved by a State’s Department of Education or a recognized accrediting organization; and
  • Completion of a coding or billing, medical record technology or health information technology (HIT) program approved by a nationally accredited agency (e.g., the American Health Information Management Association (AHIMA) or the Commission on Accreditation for Health Informatics and Information Education (CAHIIM)); or
  • Certified Coding Specialist (CCS) by AHIMA, Certified Professional Coder (CPC) by the American Academy of Professional Coders (AAPC) or Certified Medical Billing Specialist (CMBS) by the Medical Association of Billers (MAB); or
  • Two (2) years of satisfactory experience in coding and/or abstracting medical records in a health care setting.
