Overview
The Manager Social Work provides leadership to care management staff to ensure efficient care coordination, effective operations, and quality improvement. The Social Work Manager leads the execution of the Integrated Care Management Strategy. This manager partners with Integrated Care Management leaders, Cross Continuum leaders, and staff to support the ongoing development of a high performing organization. This individual will engage with clinical and operational leaders to coordinate care management programs to support sustainable achievement of the system’s Integrated Care Management objectives in support of the triple aim.

Responsibilities
Integrated Care Management:
- Collaborate with Integrated Care Management leaders to deploy the care management strategy.
- Collaborate with UPC and UPH leaders to support care management efforts and connections across the care continuum.
- Provide guidance and content expertise regarding Integrated Care Management programs and services.
- Establish and maintain processes to support the efficient and effective delivery of care management services to achieve targeted outcomes.
- Utilize data, risk stratification and other tools/reports to identify opportunities for improvement and innovation of care management strategies.
- Collaboratively develop targeted approaches to patient populations and work across UPC and UPH to connect care management approaches in the most efficient and effective manner.
- Develop and maintain appropriate training and educational resources for care management in collaboration with Integrated Care Management leadership.
- Communicate progress with care management strategies to key stakeholders.
Management:
- Directly mentors and coaches the care managers and social workers as assigned.
- Responsible for recruitment of top talent to meet program needs.
- Ensures all team members are oriented and appropriately trained.
- Implements effective performance management and staff development processes.
- Establishes effective mechanisms of communication with staff and key stakeholders to foster an environment of openness, trust, and teamwork.
- Addresses performance issues in a timely manner and according to UnityPoint Health Policies in partnership with Human Resources.
- Assists in the annual budget process.
- Analyzes and approves staff expenditures within budget parameters.
- Provide leadership and direction that ensures top of licensure duties for care management team members.
Quality Assurance and Improvement:
- Assists in establishing care management Integrated Care Management metrics
- Conducts regular operational performance analyses and engages in continuous quality and productivity improvement to promote achievement of care management objectives in support of the triple aim
- Assist with the development of education and communication related to metrics and performance improvement opportunities for care management/population health initiatives.
Partner with leaders across the continuum and health providers to identify obstacles to performance improvement and assigns resources to address them as appropriate.
Qualifications
Education: Masters Degree in Social Work Required.
Experience: At least three or more years of clinical experience with recognized advanced knowledge and expertise in the delivery of social services required. Two or more years of leadership experience preferred.
License(s)/Certification(s): Current license in good standing to practice Social work in the state where care is provided. CPR certification. Valid driver’s license when driving any vehicle for work-related reasons.
Knowledge/Skills/Abilities:
- Proficiency in use of computer applications (Microsoft Office) and reporting tools.
- Knowledge of the healthcare system and resources available to patients.
- Strong clinical proficiency and ability to apply critical decision making in dynamic situations.

Other: Use of usual and customary equipment used to perform essential functions of the position. Ability to travel to assigned regions, other UPH or community facilities. May drive a UPH vehicle, rental or own vehicle. Ability to work evenings and weekends when necessary to meet business needs of the department.
