Job Summary

The LVN Case Manager at Mission Community Hospital in Panorama City, California will work as a full-time individual contributor, responsible for providing compassionate and high-quality care to patients while acting as a liaison between the patients, their families, and other healthcare professionals. This position will report directly to the Director of Case Management and will be an integral part of the healthcare team.

Responsibilities

  • Conduct initial and ongoing assessments of patients' medical, psychological, and social needs
  • Develop individualized care plans for each patient, in collaboration with the interdisciplinary healthcare team
  • Monitor and track patient progress and provide regular updates to the healthcare team
  • Coordinate and facilitate communication between patients, families, and other healthcare professionals
  • Advocate for patients' needs and ensure appropriate resources and services are provided
  • Provide patient education on disease processes, treatment options, and self-management techniques
  • Conduct regular home visits and follow-up calls to ensure continuity of care and identify any potential issues or concerns
  • Ensure compliance with all state, federal, and hospital regulations and policies
  • Document all patient interactions and care plans accurately and thoroughly in electronic medical records

Requirements

  • Current LVN licensure in the state of California
  • Minimum of 1 years of experience in a clinical setting, preferably in case management or care coordination
  • Strong communication, organizational, and critical thinking skills
  • Knowledge of various medical conditions, treatments, and resources
  • Ability to work independently and as part of a team
  • Proficiency in electronic medical record systems
  • Bilingual (English/Spanish) preferred, but not required
