Job Summary:

Telecare Corporation is seeking a Licensed Clinical Social Worker who can provide high-quality clinical care to clients, assisting them in accessing necessary treatment and supporting them through disability claims processes. The LCSW will be responsible for guiding patients to appropriate treatment facilities, assisting with State Disability Insurance (SDI) claims, and maintaining an online account as a Licensed Health Professional for SDI.

Key Responsibilities:

• Conduct comprehensive assessments and develop individualized treatment plans for clients
• Facilitate transfer of clients to appropriate treatment locations as needed
• Assist clients in filing for State Disability Insurance (SDI) by guiding them through the claim process
• Maintain and update an SDI Licensed Health Professional online account to support disability claims for clients
• Collaborate with multidisciplinary teams to ensure continuity of care
• Provide ongoing support and counseling to help clients manage their mental health needs
  
  
  
  

Qualifications:

• Active California LCSW license
• Experience with State Disability Insurance (SDI) claims preferred
• Familiarity with managing an SDI Licensed Health Professional online account
• Strong communication skills and the ability to work collaboratively with healthcare teams
