JOB DESCRIPTION
CASE MANAGER, PERMANENT SUPPORTIVE HOUSING
FUNCTION:
Responsible for case management and coordination of services for participants in the Permanent Supportive Housing Program.
RESPONSIBLITIES:
1. To provide comprehensive, coordinated supportive services to recently homeless families affected by substance abuse in order to help them remain in permanent housing and move toward self-sufficiency.
2. To adhere to all MOHS/HUD requirements of the Housing First model.
3. Complete data entry for HMlS in a timely manner.
4. To provide case management services to up to 22 families at a time.
5. To help families become stabilized in their ability to handle concerns related to parenting, landlord issues, employment, and household management.
6. To evaluate and monitor designated services given to families in order to determine the accessibility and effectiveness of the service for the families.
7. To facilitate support groups for the families
8. To document and maintain records.
9. To work 2 evenings a week and one week-end a month.
RELATIONSHIP:
Accountable to Dayspring Housing Program Director and ultimately the Executive Director. Works as an active team member with the Dayspring staff.
QUALIFICATIONS:
1. Bachelor's Degree in social work or related field required.
2. A minimum of three year experience of counseling individuals with addictions problems.
3. Understanding of women's issues, addictions, and homelessness.
4. Knowledge and familiarity with community resources in East Baltimore
REQUIREMENTS
Maryland Driver's License.

7. Ability to work in sensitive situations and to maintain confidentiality.
8. Ability and willingness to participate in available job-related study and training which may involve occasional overnight travel away from home.

Pay: $40,000.00 - $45,000.00 per year
