Responsibilities:
* Provide case management services as part of a multidisciplinary team
* Act as the point person to mitigate housing issues as they arise.
* Develop treatment plans with consumers.
* Act as a primary case manager to consumers in accordance with Eliot's policies and PACT standards.
* Participate in completing assessments as assigned by the Team Leader or Lead Clinician in a timely manner.
* Assess client's functions, strengths and needs.
* Develop therapeutic connections with each client and provide clinical interventions.
* Provide problem-solving supports and interventions to clients
* Provide on-call on a rotating basis and rotating evening shifts.
Schedule: Monday - Friday 8am to 4pm, with one 12pm-8pm shift per week and one weekend per month. Rotating on call duties & rotating holiday schedule.
