Position Overview
As a Home Health Speech Language Pathologist, you will work closely with case managers to provide skilled services addressing speech, language, cognitive, fluency, and swallowing disorders. Your role will be central to helping patients regain or improve their communication and functional abilities within the comfort of their homes. You will serve as an educator, patient advocate, and a valued member of an interdisciplinary care team.
Key Responsibilities
- Evaluate and treat speech, language, cognitive, and swallowing disorders.
- Provide skilled therapy services in patients’ homes.
- Educate patients and caregivers on treatment plans and progress.
- Collaborate with interdisciplinary team members to ensure comprehensive care.
- Maintain accurate, timely electronic documentation.

Qualifications
- Current, unrestricted California Speech Language Pathologist license
- Current BLS/CPR certification
- Valid California driver’s license
- Strong understanding of developmental, behavioral, and cultural factors affecting care
- Home health experience highly preferred
- Experience with high-acuity patients is a plus
- Strong interpersonal, critical thinking, and organizational skills
- Ability to work independently and as part of a collaborative team
- Leadership and case management experience desired
