Key Responsibilities
Review clinical documentation, OASIS assessments, and physician orders to assign appropriate ICD-10-CM diagnosis codes for home health episodes.
Ensure coding is in compliance with CMS regulations, agency policies, and current coding guidelines.
Collaborate with clinicians, QA staff, and physicians to clarify diagnoses and documentation as needed.
Perform pre-bill audits and verify that documentation supports the assigned codes and patient care plan.
Maintain up-to-date knowledge of coding guidelines, OASIS-E data elements, and regulatory changes affecting home health.
Support accurate HHRG (Home Health Resource Group) and PDGM (Patient-Driven Groupings Model) assignments.
Assist in training new staff and clinicians on documentation requirements and coding best practices.
Meet coding turnaround times and productivity standards as set by the organization.
