The Position

We are seeking a full-time Group Case Manager for our Balboa office in Northridge. This position reports directly to the Group Services Coordinator. As a crucial member of the Access to Care Division, the Group Case Manager collaborates with Care Coordination Team (i.e. Case Flow Coordinator, Admission Specialists, and Therapists) to ensure that families obtain care in a timely manner.

Essential Job Functions

  • Monitor group attendance and proactively follow up with individuals who miss sessions.
  • Notify clinicians of cases that require closure or follow-up action.
  • Collaborate with the billing department to ensure accurate and timely billing for group services.
  • Maintain up-to-date and accurate group participant lists.
  • Work closely with team members to identify appropriate referrals for group services.
  • Provide clear guidance to clinical staff on necessary documentation for case transfers or closures.
  • Actively participate in staff meetings, sharing relevant information and updates about group services.
  • Collect and maintain data for case flow and service reporting.
  • Facilitate assigned group therapy sessions in alignment with program goals and client needs.
  • Use collaborative documentation practices to prepare session notes and billing, ensuring compliance with DMH standards for linkage interventions.
  • Drives own vehicle as needed.
  • Starting salary: $20.50 per hour
  • Hours: Monday - Friday 8:30am - 5:30 pm, one-hour meal period (some evenings may be required)
  • Fully onsite to start. Hybrid options become available after 90 days of successful employment.


Qualifications

  • Bilingual in Spanish and English, with strong communication skills in both languages.
  • Demonstrates professionalism, calmness, and a positive demeanor in challenging situations.
  • Maintains a mature, professional attitude and excellent judgment, knowing when to consult with a supervisor.
  • Strong organizational skills with the ability to prioritize tasks, work independently, and meet deadlines.
  • Committed to confidentiality, accuracy, and continuous professional development.
  • Excellent interpersonal skills and the ability to collaborate effectively in a team-oriented environment.
  • Consistently punctual, with a strong attendance record and reliable performance.
  • Enthusiastic commitment to the department’s goals and the overall mission of the Center.
  • B.A./B.S. needed in Mental Health, Psychology, Social work, child development, or other related fields
  • Minimum of 2 years of experience in working with children and families.
