About the RoleWe are looking for an experienced Exercise Physiologist to take on a part-time role at Patches, where you will deliver transdisciplinary therapy services to a diverse caseload, including paediatric, adult, elderly, and disability clients. This role offers a range of client visits and community-based interventions, providing the chance to work with individuals from culturally and linguistically diverse backgrounds.
This position requires travel to regional locations: you will travel to the Wheatbelt (DIDO) for two days fortnightly and to Port Hedland every six weeks for a three-day trip. You'll have the opportunity to contribute significantly to our clients' well-being while also advancing your professional development.
A typical day in the role may look like:\n
- Working within Patches' main office in Subiaco, seeing clients face-to-face or via telehealth.

- Conducting home visits
\n
- Traveling to the Wheatbelt and Port Hedland for trips to assist regional participants.
\n
- Collaborating with a multidisciplinary team of clinicians to complete therapy summary reports, discuss participant goals, and deliver joint therapies.
\n
- Completing therapy notes, reports, and other documentation related to client care.
\n

Key Requirements:\n
- A minimum of 2 years’ experience working as an Exercise Physiologist, particularly in the disability, paediatric, or aged care sectors.
\n
- Experience in providing services within the National Disability Insurance Scheme (NDIS), including assessments and developing treatment plans.
\n
- Strong skills in conducting home assessments and providing recommendations for home modifications and equipment prescriptions.
\n
- A passion for working with a varied caseload, including children, adults, and aged-care, in both metropolitan and regional areas.
\n
- Relevant professional registration and qualifications.
\n
- National Police Clearance (within 6 months)
\n
- Valid Working with Children Check
\n
- NDIS Worker Screening Check.
\n
- Australian Driver’s License
\n
- Proof of work rights (Australian Passport/Citizenship Certificate or valid visa).
