Job Status: Non-Exempt
Position Summary: $17/hr. Monday to Friday 7am to 3pm
The Direct Support Professional is responsible for a variety of duties related to the overall care of consumers residing in community residential settings. This includes direct care of program individuals and implementation of individual support plans, all client related activities, documenting consumers progress and responding to program service needs while ensuring the safety and well-being of the consumers. Direct Support Professionals are required to perform all duties in accordance with Minnesota statues 245D Home & Community-Based Service Standards.

Active Treatment Services
1. Carries out assigned program teaching plans as scheduled
2. Collects factual habilitation data; measure and record behaviors and/or skills by accurately completing the data collection and baselining forms.
3. Carries out program plans as specified and implements therapeutic interventions as needed within the procedures
4. Obtains as working knowledge of consumer(s) history, skills, areas of vulnerability, habilitation plans and emergency procedures
Daily Programming & Activities of Daily Living
5. Ensure a safe, clean and secure living environment for all consumers by assisting them in house cleaning tasks and completing the tasks if necessary, by the end of each shift
6. Follows daily routine while allowing for flexibility and planning creative alternatives
7. Assist in the development and adherence to healthy menu(s) (according to client’s dietary needs) and in the preparation of meals as indicated by the client’s individual skill level and individual program plan.
8. Assist consumer with their daily routine as needed, including but not limited to hygiene functions & grooming, leisure time usage, socializing, budgeting and money usage, safety and health, and participating as a member of the community
9. Documents and accounts for all agency and consumer related expenditures according to prescribed procedures
10. Encourages and assists in the usage of public resources and outside recreational activities in an integrated setting
11. Promotes participation and involvement in community outings
12. Provides transportation to and from activities
13. Assists in the implementation of individual plans for individuals and behavioral plan goals
14. Assists individuals in the pursuit of recreational, social and community activities based on their preferences and individual plans; presents creative options as needed
15. Assists and encourages individuals in the establishment of social networks
16. Works all assigned schedules, including but not limited to: arriving and departing on time, requesting time off in advance, and notifying your supervisor of changes to your assigned work schedule
17. Act in accordance with the agency’s Policy and Procedures manual including policies concerning consumer rights.
18. Treats individuals with dignity and respect
19. Complete household cleaning duties as designated on the cleaning list
Communication and Development
20. Obtains a working knowledge of program and organizational policies and procedures
21. Accurately and objectively document all client documentation before leaving shift, including, but not limited to, shift notes (detailed information on each client), attendance reports, communication and medication logs, incident reports, and outcome data sheets. Documentation must meet agency accreditation and funding standards.
22. Documents individual’s health (i.e. seizure charts, weight, bowel movements, fluid intake.)
23. Ensures Medication Administration Record (MAR) entries are completed daily
24. Read and initial staff communication log at the beginning of each shift.
25. Participates as a team member and offers suggestions, voices concerns, and provides information regarding consumers program
26. Develop and maintain professionally appropriate therapeutic relationships with each consumer and the consumer's family.
27. Maintain positive working relation with staff and other related persons to ensure services are delivered in a smooth and effective manner.
28. Maintains consumer confidentiality
29. Observes individuals for evidence of injury or bruises and evaluates for changes in emotional and physical status
30. Advise supervisor, coordinator of any problems which arise during the shift (i.e., problems regarding consumers, physical plant, and or personnel issues).
31. Keeps supervisor and coworkers informed of all services-related unusual and emergency occurrences and complete written incident report form
Health & Safety
32. Maintain appropriate staffing ratios as directed
33. Ensures the safety and well-being of all consumers
34. Reports unsafe conditions, environment, and equipment to Program Coordinator
35. Conduct and document fire drills as assigned by supervisor.
36. Administers medication as needed after obtaining required training
37. Assures that each consumer has nutritionally adequate meals available at regularly scheduled times
38. Maintains equipment or household as assigned
Continued Development
39. Attends staff meetings and other meetings on a consistent and timely basis
40. Attends all required training sessions to increase skills and knowledge and to meet all licensing requirements; share that information with other staff as appropriate
41. Maintains all employment standards, required training and licenses
Working Conditions:
· Is subject to flexible hours including split shifts or varied days and times including mornings, afternoon, evenings, weekends and overnights depending on coverage needs
· Regularly travels in the community
· Works in a variety of settings inside and outside the home and throughout the community
· Is subject to frequent interruptions and imposed deadlines
· Sits, stands, walk, bends, lifts and moves intermittently during working hours
· Is subject to frequent problem-solving, activities
· Has regular contact with consumers, staff, family members, and the general public
· May be subject to hostile and emotionally upset consumers, staff, family members and the general public; may be subject to physically aggressive consumers
· Attends and participates in a variety of educational programs designed to increase knowledge and expertise in the field
· May be exposed to infectious diseases, odors, dust, etc. throughout the day
Minimum Qualifications:
1. Be at least 21 years of age or older.
2. Have a high school diploma or GED.
3. Must be able to read, write and speak the English language at a level that meets the performance requirements.
4. Possess a valid driver's license and; depending on site, possess personal vehicle for use in performing work related activities, and maintain necessary insurance coverage.
5. Pass a background check and receive fingerprint clearance from the Department of Human Services.
6. Be physically able to perform evacuation of the building, handling aggressive client situations and medical emergencies, including the ability to lift at least 50 lbs, being able to ambulate quickly, and evacuate the building within 3 minutes.
7. Possess good health and demonstrate emotional stability.
8. Willingness to work in a variety of settings and with a variety of level of care needs without direct supervision.
Preferred Qualifications:
1. Experience in providing support to families.
2. Successful completion of post high school education in a related field.
3. One-year experience working in the provision of habilitation services to individuals.
4. A proven ability to work with individuals with extreme behavioral and/or physical needs.
5. Six months experience in the implementation of positive behavior management techniques.
