Job Description

The Care Coordinator will serve as a vital link between patients, healthcare professionals, and internal departments to ensure smooth coordination of care and accurate documentation. The ideal candidate will possess a background in insurance, intake, scheduling, and administrative support within a medical office setting.

Key Responsibilities

  • Collect and verify client information including demographics, insurance coverage, and provider details
  • Maintain and update patient tracking logs to monitor progress and communication
  • Coordinate with external partners such as physician offices, therapy providers, billing departments, and clients to ensure timely service
  • Manage incoming documentation and organize electronic and physical files accurately
  • Schedule services and facilitate the flow of necessary paperwork across departments
  • Provide general office support, including greeting clients and answering incoming calls
  • Follow up with clients to confirm receipt of documents and completed orders
  • Safeguard confidential client records and ensure proper closure of secure file rooms
  • Collaborate across departments and lend support to various internal teams as needed

Qualifications

  • Experience with complex rehab technology (CRT) or durable medical equipment (DME) REQUIRED
  • High school diploma or GED required
  • Background in administrative support, scheduling, or customer service within a healthcare environment
  • Familiarity with collecting patient demographics and handling sensitive documentation
  • Proficiency using Google Workspace (G-Suite), Adobe, and medical tracking systems similar to Bright Tree or Atlas

Additional Details

  • Hours: Monday – Friday, 8:00 AM to 5:00 PM
  • Assignment Type: Contract-to-Hire (720 hours)
  • Free onsite parking
  • Business casual dress code; professional jeans permitted

  • Hands-on and virtual training provided (approximately 1+ month)
  • Reporting to: Care Coordinator Manager and Clinic Manager
