Summary

JOB SUMMARY: The Hospice Patient Care Coordinator participates in all clinical management functions. Coordinates the activities of the interdisciplinary team in the various patient care settings. Responsible for clinical direction of all members of the interdisciplinary team with assistance of discipline specific team leaders. Responsible for staffing including the hiring and orientation of staff to assure adequate coverage to meet patient needs, in accordance with the Human Resources Director. Assures compliance with all state and federal regulations. ESSENTIAL FUNCTIONS: 1. Interviews, hires, orients and assigns appropriate staff to assure patient needs are met in conjunction with the Human Resources Director. 2. Provides staff with ongoing education and clinical direction. 3. Supervises, evaluates and counsels members of the interdisciplinary team as appropriate. 4. Participates as a member of the management team and supports organizational decisions. 5. Manages and reviews the budget for errors and cost containment for the assigned team. 6. Participates in QAPI functions. 7. Ensures the patient record (chart) is kept up-to-date and contains all required components. 8. In charge of patient admissions and referral process. 9. Other duties as assigned. QUALIFICATIONS: Education/ Degree: Graduate from an accredited school of nursing, BSN preferred. Training/Licensure: Currently licensed as a registered nurse to practice in the state. Knowledge/Skills/ Abilities: Ability to work independently, make accurate and, at times, quick judgments. Ability to supervise others appropriately. Ability to respond appropriately to crisis outside of a hospital setting. Acceptance of and adaptability to different social, racial, cultural, and religious modes. Completes Hospice of Kimes training program. Professionalism in interpersonal communication skills with patients, patient families, colleagues, physicians, and other healthcare professionals. Personal professional development, accountability, organization and leadership in the performance of professional nursing practice. Experience: Minimum 3 years clinical nursing experience required. Supervisory experience preferred. JOB FACTORS:Physical Requirements: Requires considerable physical effort most of the day including kneeling, squatting, reaching, twisting, climbing, walking, exposure to temperature and humidity changes and maximal assist in lifting and/ or transferring of 100 pounds. Must possess sight/hearing senses or use appropriate adaptive devices that will enable senses to function at a level required to meet the essential duties of the position. Must provide evidence of annual TB test and other state required tests or examinations. Mental Requirements: Must be able to work independently, make judgments based on assessments and data available and act accordingly. Must be flexible, innovative, and possess good interpersonal skills. Must be able to cope with mental and emotional stress and demonstrate emotional stability. Working Conditions: Be able to tolerate exposure to elements including, but not limited to, odors, blood, body fluids and excrements, adverse environmental conditions, and hazardous materials. Transportation: Must have a current valid driver's license, auto liability insurance, and reliable transportation.
