Compensation Range: $19-21/hourly commensurate with experience

Summary

The Medical Assistant is a member of the medical care team and is responsible for assisting nursing and medical provider/staff with a prescribed set of duties in the provision of patient care. This position also performs clerical duties, acts as receptionist and communicates with all disciplines. This position supports Housing Works mission to connect new and existing clients/patients to programming through an efficient and friendly customer experience. The Community Digital Navigator works as an Extender to the Account Management team and provides a warm welcome for the clients the Account Managers are connecting to Housing Works. The candidates for this position will need to be comfortable using a telehealth platform such as Healow or Doxy and will need to be able to coach patients through network issues and common obstacles that may be faced when using telehealth. The Community Digital Navigator will also be responsible for notetaking, registering patients into the Electronic Medical Record (EMR), creating, and updating patient charts, scheduling follow-up visits, and more. The Community Digital Navigator will travel to different off-site locations where the service will be provided with the possibility of remote work as the project progresses. The person in this position would have to be sensitive, flexible, and responsive to gender, race, ethnicity, socio-economic status, religion, age, sexual orientation, or other special needs as reflected in the ability to communicate with staff and/or clients. They would also have to uphold and advance the values, ethics, knowledge, and mission of Housing Works.

Primary

Essential duties and responsibilities:

  • Schedule patient visits in EMR.
  • Reach out to patients to confirm/cancel appointments.
  • Greet and register all patients as they arrive, perform insurance verification/preparation, schedule appointments, answer calls, take messages and other similar clerical tasks.
  • Collect patient information and documents appropriate notes in EMR.
  • Coach patients through technological issues, network difficulty, how to use telehealth platform.
  • Maintains timely visits between patients and providers and keeps provider on schedule Maintains standard precautions in the provision of direct patient care; obtain vital signs before visits- including height, weight, vision screening, glucose tests, peak flow measures, etc.
  • Assist in procedures that are appropriate relative to the Company’s policy and/or general clinical operating standards as requested – EKG, etc.
  • Verbalizes knowledge of and adherence to OSHA standards for blood- borne pathogens;
  • Utilizes appropriate technique in performing venipuncture to obtain blood specimens in accordance with agency procedure.
  • Responsible for all Lab functions – clinic liaison for lab issues, performs phlebotomy, replenishes lab supplies, etc. Responsible for performing and documenting CLIA controls as indicated, specimen refrigerator and lab room temperatures daily.
  • Ensure and maintain an adequate inventory of all necessary supplies, including medical equipment, forms and other inventory that might be appropriate or required by the physician.
  • Completes or assists in the completion of various patient forms such as insurance claims, disability forms, medication approvals, etc.
  • Coordinates discharge process - schedules, all f/u appointments, referrals, radiology, transportation, prior approvals, etc.

Secondary

  • Organizes and prepares patient summary sheets and charts for daily clinic.
  • Organizes patients to see the provider.
  • Obtains chief complaint from patient.
  • Understands and maintains universal precautions in all clinical activities.
  • Maintain knowledge of Housing Works' programming, screening tactics and eligibility requirements as well as effectively articulate the benefits to eligible clients/patients.
  • Provide follow-up support to ensure client/patient adherence to referrals made including follow-up calls and/or emails to Partner/Case Manager
  • Document and critically think through possible solutions for customer service issues.
  • Assist team in organizing outreach efforts, promotional materials and producing data reports that help improve quality of partnership.
  • Participate in professional training and development opportunities including certifications that may be required.
