This person will provide program oversight involving compliance with licensing and is encouraged to express innovative ideas on our programming and the overall satisfaction of the individuals we support and their support teams. Full Time Position – Monday through Friday (days) occasional nights, weekends, & rotational on-call responsibilities. Pay: $21+/hour + benefits package. We will pay a salary package which is commensurate with experience after the probationary period. ** DUTIES AND RESPONSIBILITES Evaluation of effectiveness of service delivery, methodologies, and progress on the person' outcomes based on the measurable and observable criteria. Ensure that outcomes are appropriate, and person centered. Monitor/Evaluate documentation daily and provide training to staff as needed to ensure consistency in documentation. Managing services, which may be hourly or 24-hour supports Report and document incidents and emergencies, and other necessary reporting Working alongside the HR team to communicate with individuals, guardians, case managers, behavior analysts, and other team members to decide on services needed to support each individual most effectively Working direct care (minimum of 24 hours per week) Ensures compliance with each person served with all 245D paperwork including CSSP, CSSPA etc.. Coordinating appointments for individuals as requested Develop, document, and nurture annual plans with the people you support and their team Update team members for each person served at minimum of once per month about status of service, quality of care, person situation and outcomes. Other duties as assigned. QUALIFICATIONS FOR THE JOB Education: Need to meet one of the following qualifications: 1. Bachelor's degree in a field related to human services, and one year of full-time work experience providing direct care services to persons with disabilities or persons aged 65 and older. 2. Associate degree in a field related to human services, and two years of full-time work experience providing direct care services to persons with disabilities or persons aged 65 and older 3. Diploma in a field related to human services from an accredited postsecondary institution and three years of full-time work experience providing direct care services to persons with disabilities or persons aged 65 and older Experience: To qualify for the position, applicants must: Pass a Department of Human Services background check and provide three verifiable professional references Have a valid driver's license Work independently and have strong organizational skills Demonstrate strong written and oral communications skills Proficiently use Microsoft Office and Google Docs Applicant must have 3+ years of full-time work experience providing direct care services to persons with behavioral health concerns AND at least 3+ years of management experience as a Designated Coordinator or similar role.
